Consent and Assent Forms

We have the utmost respect for students, parents, teachers, administrators, schools, and communities who participate in this project.  We have gone to great lengths to ensure that participation is consented by all parties. We have made sure that we have communicated the scope of this project to all participants. That said, we remain open to communication about participants thoughts and feelings.  Recognizing that this is a community affairs project, we want to ensure that everyone is safe and happy with their place in this work.
